Students requesting academic accommodations for courses need to complete the request for Accommodation Verification Letters form linked below.  

Letters are prepared by the Office of Disability Services and emailed to the student.

Once you have the letters you should print a copy for each of your faculty members and present the letter to them after class or during office hours.  Accommodation verification letters are valid for the entire academic year and can be used for Fall, Winter, Spring, and Summer classes.
