Forms Please utilize the forms below for requests related to dropping or withdrawing from a class, requesting an independent study, requesting an incomplete, declaring a class as grade/pass/fail, and submitting an academic petition. Course Drop or Withdrawal Form Request for Independent Coursework Username = <Your portal username>@curry.edu (ex. acurry2020@curry.edu) You do not need to include "students". This is not the same as your email address. Password = Your portal password A student may request that the course instructor grant an incomplete because of an extenuating circumstance. The student must be passing the course at the time of the request in order to qualify for an incomplete. The choice to grant an incomplete is the prerogative of the instructor. An incomplete cannot be requested beyond the last day of classes in the semester (not including finals). Please refer to the academic calendar for Grade/Pass/Fail request submission deadlines. You may select one course per semester to take G/P/F. The course may not be in your major. You are permitted a maximum of four G/P/F attempts during your time at Curry. Please see the Minimum Letter Grade section for more information on how it works. NOTE: To be eligible for Dean's List, you must have a minimum of 12 graded credits in the semester, in addition to meeting the minimum 3.30 semester GPA. A course in which you earn a P grade does not count toward your graded credits. This form is used by students requesting a waiver or relief from an academic policy or deadline for extenuating circumstances. |
