Harvard Pilgrim Health - Online Enrollment Instructions: Eligible employees can follow these Step by Step instructions to enroll in one of the Harvard Pilgrim Health Plans.

Harvard Pilgrim Health Plan - Enrollment Form: Please use this form to enroll in the Harvard Pilgrim Health plan, or to make changes to your existing Health plan (i.e. going from an Individual plan to the Family plan or vice versa).

Harvard Pilgrim Fitness Reimbursement Form: Harvard Pilgrim Fitness Reimbursement Form. Open and read the instructions, then proceed to fill out the Fitness Reimbursement Form, print it and mail it in.
