It is the responsibility of the College and its employees to ensure that its educational programs and other activities protect and promote the health and safety of our students, our employees, and the environment. Public Safety offers a number of training sessions throughout the year to address community awareness and safety, including visiting each department to present the video "When Lightning Strikes" to address what to do if a shooter is on campus. NOW OFFERING AN IMPORTANT RESOURCE THAT ALL MEMBERS OF THE CURRY COMMUNITY SHOULD ACCESS:
Title IX of the Civil Rights Act requires that the College provide training on sexual assault and harassment procedures to all staff and faculty members. We therefore ask that you take the time (approximately 45 minutes, which can be started and stopped as time permits) to complete an on-line training. This training was developed by our liability carrier, United Educators, and was selected and successfully "beta-tested" last semester by a number of staff and faculty members here at Curry.
Our goal is to have all members of our community participate in this brief online course, "Preventing Workplace Harassment". This will be another important step towards building an inclusive Curry community that is free from harassment and discrimination.

You will need to follow the web link in step one below that brings you to UE's Learning Management System (LMS) and our institution's registration code that validates your eligibility to take the course. Once the registration code is accepted, you can register for and take the "Preventing Workplace Harassment" course by selecting the one that is appropriate for your role here at the college (Faculty, Staff, etc.). Click here for a helpful overview of how to access and register for the United Educators LMS course.
Please follow these instructions to access UE's online course*:
- Click on the link, https://elearning.ue.org/
- First-time users select the link on the right side of the screen to create a new account
*Use this institution registration code: 0585-SC37-XY12
*Provide specific information about your role at the institution and then create a username and password
- Use your username and password each time you log in to the system using the link provided in Step 1 above
- Select the "Preventing Workplace Harassment" course by selecting your correct college role under the course name (Faculty, Staff, etc.)
- Begin the course with Module 1, "Introduction"
- Once completed with all modules please select "certificate" link to print your certification of completion. This action will trigger a completed status in the system for you. (This link can be found at the end of the module menu).
*Please note: if a user already has a UE issued website account they do not need to use the registration process outlined above as they already have admittance to the courses as part of that access level. The following employment documents are necessary for Curry to maintain up-to-date and compliant records and information on our employees. Most of the forms below are available in electronic format so that you may type in the content. All documents are to be submitted to the Office of Human Resources. If you have any questions, please contact us.
Employment Notice (page one) - All new Faculty and Staff Employees are to complete an Employment Notice and submit the completed form to Human Resources.
Self Disclosure Form - To help us comply with federal government and accrediting association record keeping requirements, and in order to effectively monitor the success of our recruitment and employment efforts, it is requested that you complete this form. The data on this form will be kept in a confidential file and is not part of your application for employment or your personnel file. Your cooperation in completing this form is appreciated yet is voluntary.
Form I-9 Federal Employment Verification Form for all new employees - The purpose of this form is to document that each new employee (both citizen and non-citizen) hired after November 6, 1986 is authorized to work in the United States.
The Federal Form I9 is nine (9) pages and includes several pages of instructional material, two (2) pages of the form to be filled out by the employee and Human Resources, and the federal list of acceptable documents. Every newly hired employee (including students, faculty and staff) should follow these steps:
- Please review the employee instructions (pages 1-2) of the Form I9 by clicking here.
- Complete the Employee Section One of the I9 Form (PDF) on your first day of employment. Click here to access the form. You may complete it on the computer but it must be printed after completion and cannot be saved.
- Bring your completed Form I9 to Human Resources along with your required unexpired original identification*. For information on what identification is acceptable please click hereto review or print the Federal List of Acceptable DocumentS (page 9 of the Form I9).
*New employees need to bring their identification documentation to Human Resources no later than three (3) days after your date of hire. No employee can work or be processed in payroll until Human Resources has validated the employee's identification documents.

W-4 Form-New Employees are to complete a W-4 on the first day of employment so that the College can withhold the correct federal income tax from your pay. Complete a new W-4 form when your personal or financial situation changes and in order to make changes to your payroll tax deductions. Completed forms are to be submitted to Human Resources. Address/Personal Change Notice - The Address / Personal Change Notice should be completed whenever an employee needs to change their personal information such as name, address, telephone number, or emergency contact information. Payroll Calendar – Here you may access a complete listing of the bi-weekly payroll cycles, and pay dates. Direct Deposit Form - Full Service Direct Deposit automatically deposits your paycheck into the bank account(s) you select. Distribute your pay among multiple accounts (checking, savings, Christmas clubs, investment accounts etc.) at different financial institutions. You won't have to stand in long check cashing lines to deposit your pay anymore. Your pay will be in your account(s), ready for immediate use - even if you can't get to the bank.
Employee Handbook - This Handbook is designed to provide an overview of many of the employment policies, procedures and benefits for staff employees at Curry College who are working in the areas of Administration, Clerical, Buildings and Grounds, and Public Safety.
Accident Report - Please complete this form if you have had any type of accident or injury at work. Send the completed form to the Human Resources office within 24 hours of the incident.
Employment Notice (page 2) - The new hire completes page one of the Employment Notice (available above) while the hiring manager completes page two of the Employment Notice provided here. This notice provides all the important hiring detail necessary for HR and Payroll to set up the new employee in our system.
Change Notice -Change notices should be completed when there has been a change in job title, and/or salary information.
Termination Notice - Supervisors are asked to complete this form for any employee who has terminated employment with the College, and submit the completed form to Human Resources.

MANAGERS & FACULTY HIRING STUDENT WORKERS
STUDENT EMPLOYMENT PROGRAM FORMS & PROCESS
Step 1: Complete aStudent Job Requisition Formand submit to HR to post your open positions on CurryConnect. All Students are being directed to CurryConnect to find jobs.
Step 2:Receive Student Employment Applications for consideration and go through your selection process.
Step 3:Verbally confirm your hiring decision with your selected student worker. For ALL student hires please direct them to the student portal and Human Resources to complete important paperwork BEFORE they start working.
Step 5:Send to HR the "Hiring Packet" consisting of the following:
- Application (and any other submitted candidate material s such as resume, cover letter) - New hires only
- Student Personnel Action Form (S-PAF) – ALL Student Workers
NOTED Changes to the Student Employment Process
Only the Student Personnel Action Form (S-PAF) is now being used to to process student hires - including new and returning, work-study and non-work-study.
Work-study contracts no longer need a manager's signature and are handled between the student and HR directly. Please direct the student to the portal for instructions regarding their work-study contract.
To connect with prospective student hires send HR the Job Requisition form (available above) to post your position on CurryConnect. All students are being directed to the CurryConnect posting to identify opportunities for campus work.
Student Employment TOOLS AND TIPS
For your convenience and to assist you with the steps of the process, a few new tools are now available. Feel free to save these forms to your computer:
- Some managers requested a checklist - the Manager's Checklistfor hiring student employees – please let us know if this meets your needs.
- A chart is also now available to provide a quick one page summary of the cross-functional process and the necessary steps to employ a Curry student on-campus.
- In addition, a Candidate Interview Feedback formwas requested and is now available to assist managers in documenting the interviews conducted.

ENROLLMENT FORMS AND RATES
Dental Plan - Enrollment Form: Please use this form to enroll in the Curry College Dental plan, or to make changes to your existing Dental plan (i.e. going from an Individual plan to the Family or vice versa).
Health and Dental Rates: Medical and Dental Premiums and Employee Cost for the Benefit Year beginning 4/1 through 3/31.

The Dependent Care Assistance Plan gives you the opportunity to have money withheld from your paycheck on a pre-tax basis for expenses related to the care of your children or other dependents so that you are able to work. The maximum contribution is $5,000.00.
Enrollment Form - Faculty and Staff who are interested in enrolling in the Dependent Care Assistance Plan must complete a new enrollment form each plan year.
Plan Summary - Curry College maintains the Curry College Flexible Benefits Plan for Dependent Care Benefits (the "Plan") for the benefit of its eligible employees. The terms of the Plan are contained in a lengthy, legally worded document. This Plan Summary is intended to acquaint you with the provisions of the plan that apply to you by summarizing them in a language that is easier to understand.
Account Information - This informational Q&A sheet was developed to answer some frequently asked questions regarding the Dependent Care Assistance plan.
Reimbursement Form - Please complete and submit the Dependent Care Assistance Plan Reimbursement form, and all supporting documentation to the Curry College Finance Office.
The Medical Reimbursement Plan is a medical expense reimbursement account which allows you to have money withheld from your paycheck on a pre-tax basis for medical, dental, and vision expenses that are only partially covered, or not covered at all by your health care plan. The minimum contribution is $100.00, and the maximum contribution is $2,500.00.

Saving for retirement is an extremely important endeavor for all of us. Curry College provides a very generous plan for benefits-eligible staff and faculty members, and we also provide a tax-deferred savings vehicle for many employees who are not otherwise benefit-eligible.
The Curry College Defined Contribution Retirement Plan provides you with an opportunity to save for your retirement, while saving on your current income taxes. Below are links to useful documents that help to describe some of the benefits available to benefits eligible staff members.
Curry College 403(b) SPD -Curry College is proud to offer the Curry College Defined Contribution Retirement Plan (the "Plan"). The Plan is offered pursuant to Section 403(b) of the Internal Revenue Code. This Summary Plan Description is a brief summary of the Plan.
The first portion is for the Retirement Annuity (RA) Contract. This is the account for your "Basic Contribution" as well as the College's Matching Contribution.
The second portion is for the Group Supplemental Retirement Annuity Certificate (GSRA). The TIAA-CREF Supplemental Account is for any contributions exceeding the maximum "Basic Contribution", OR for any contributions made prior to your Eligibility for the College's matching contribution.
Faculty Salary Reduction Form - To make adjustments to your current contributions to the Curry College Retirement Plan or if you are enrolling in the Retirement Plan for the first time, please complete the Salary Reduction form. All completed forms should be returned to the Human Resources office a minimum of 2 weeks prior to the effective pay-period start date.
Staff Salary Reduction Form - To make adjustments to your current contributions to the Curry College Retirement Plan or if you are enrolling in the Retirement Plan for the first time, please complete the Salary Reduction form. All completed forms should be returned to the Human Resources office a minimum of 2 weeks prior to the effective pay-period start date.
Investment Policy Statement - This statement establishes policies adopted by the Curry College Retirement Committee for the investment of the assets of the Curry College 403(b) plan.
Monitored Investments - The Retirement Plan Investment Committee continues to meet quarterly with our independent investment advisor, the Bostonian Group, to review the investment performance of the monitored funds in our Fidelity and TIAA-CREF offerings. This document includes a list of those funds.
Retirement Calculation Worksheet- Want to quickly find out how much of your Salary you can contribute to your 403(b) Retirement plan? Use this worksheet to help you determine what percentage of your pay you will be withheld, based on the dollar amounts you enter.
Loan Policy Statement - This is an explanation of the rules for taking a loan from your Curry College 403(b) Defined Contribution Plan.
Hardship Withdrawal Application - Employees requesting a Hardship Withdrawal from their Curry College 403(b) Defined Contribution Plan should first contact the Benefits and Compensation Administrator, or the Director of Human Resources to discuss their eligibility for the withdrawal.
If it is determined that an employee may meet the eligibility criteria, they will be asked to submit the above mentioned application, as well as all supporting documentation indicating the need for the hardship withdrawal.
FAMILY MEDICAL LEAVE ACT (FMLA)

OTHER PLANS/PROGRAMS
Employee Assistance Plan - No matter what the issue, Lincoln Financial Groups Employee Connect services can help you with confidential support, guidance, and resources 24 hours a day,7 days a week.
LifeKeys™ Program - Employees who are insured under the College's Life and LTD Insurance program, as well as their dependents are eligible to use Lincoln Financials LifeKeys program. The LifeKeys program provides online access to a variety of resources. Automobile Insurance Discount - The Insurance Commissioners' office for the State of Massachusetts has approved the discounts for Group Automobile Insurance. The Employees of Curry College group will receive an 5% discount through the year for Automobile Insurance policies issued with Safety Insurance through the Safety Net Program.
Curry College is proud to participate in the Tuition Exchange (TE) and Council of Independent Colleges (CIC) programs for undergraduate education of dependent children. Additional Information on each of these programs can be found below.
TUITION EXCHANGE (TE) PROGRAM
The Tuition Exchange opens doors and creates opportunities through a reciprocal educational scholarship program. Their awards enable the dependents of faculty and staff at participating colleges and universities to significantly defray their higher education costs.
The Tuition Exchange scholarships are competitive awards and are not guaranteed. On average, 6,000 scholarships are warded annually, with amounts averaging about $24,000. For more detailed information about the Tuition Exchange please visit their website at www.tuitionexchange.org
THE CIC TUITION EXCHANGE PROGRAM
The CIC Tuition Exchange Program (CIC–TEP) is a network of CIC colleges and universities willing to accept, tuition-free, students from families of full-time employees of other CIC–TEP institutions. Each participating institution in the network agrees to import a limited number of students on the same admission basis as they accept all other students, without regard to the number of students it exports.
With a current membership of nearly 400 participating institutions in CIC–TEP, participation is at its highest level yet, allowing for a large selection of institutions.
